End-of-Life Care Options
Creating Individualized Plans for Every Patient in Los Angeles
While end-of-life can sound negative, hospice care is not about giving up hope. It is about providing comfort and support for both patients and families during a patient's last days. At Mid Valley Hospice, we are passionate about enhancing the quality of each patient's life while supporting their family members or loved ones during the dying process.
We act as a hospital on wheels, coming directly to your loved one's location throughout Los Angeles County to provide our personalized end-of-life care for patients who are certified to receive hospice care from their primary physician.
Some of our end-of-life care services include:
- Medication assistance
- Equipment and supplies for management of terminal illness
- Personal care, such as wound dressing, mobility assistance, etc.
- Palliative care
- Comfort care
Who May Need Our Care?
Our end-of-life care is designed for patients who are in need of care to allow them to die with dignity and an improved quality of life. We address their various medical needs, as well as provide emotional and spiritual support through our other services when desired. We also provide support for families during this time, helping them accept the end-of-life process.
Specifically, Mid Valley Hospice offers end-of-life care terminal illness whose life expectancy is measured in months rather than years with a focus on comfort-oriented care, though some life-prolonging treatment may be used if a worsening condition is preventable.
We commonly provide end-of-life hospice care for those with:
- End-stage heart and lung disease
- Kidney disease
- ALS and other neurological diseases
- Alzheimer’s disease
- Other life-limiting diseases
Our interdisciplinary team includes on-call doctors, registered nurses, volunteers, social workers, certified hospice aides, bereavement counselors, and other professionals who are ready to assist your loved one and your entire family. We meet regularly to ensure our plan of care is still appropriate for each and every patient we serve.
We are available 24 hours a day, 7 days a week to come to your loved one and provide the end-of-life care they need, whether they are in Sherman Oaks, Glendale, or the greater Los Angeles area. Call (818) 403-3940 if you would like to learn more.
